“When our daughter was born with medically complex needs requiring the coordination of numerous specialties we felt completely overwhelmed trying to manage her care in the NICU. Since being discharged we have been extremely fortunate to have access to care coordination through the Little Hero’s League. After meeting our care coordinator we felt an immediate sense of relief knowing there is someone to help us navigate the health care system and advocate for our daughter’s needs. In the short time we have been part of the program our care coordinator has set up and attended meetings with our doctor in an effort to reduce emergency room visits. Care coordination is something that our daughter and family will benefit from greatly while we organize multiple surgeries, appointments, and potential setbacks during our daughter’s first year of life.”

“My son was in NICU for 8 months with wonderful follow up from the staff there the whole time. We finally made it home and there was so many things that I hadn't even thought of like who to call, when to go to the hospital, how to find the right place for your little one’s appointment. In NICU we always had someone to answer our questions and I felt a bit lost in the chaos of it all at home. That's when our care coordinator, Lisa, called. My care coordinator checks in frequently with our family to make sure we have the information needed to take care of our son. For example, if I need a phone number to a department, but have no clue how to get it. Or if we are at Lurie's, but I don't know how to get to his appointment we now have her there to call on. Right now, Lisa has been with us for about a month and there are so many situations that have come up already that I could not have foreseen. She has been to our home and to several appointments. It really helps to have someone on your team. It made the transition home from NICU a lot easier for us.”
